Passport for life is a useful to to give students, parents, and teachers an idea of the child's health literacy. The video below explains what the benefits of using this tool are.

The areas covered by his tool are Active Participation, Living Skills, Fitness Skills, Movement Skills, and Goal Setting. My favourite aspect of this assessment tool is the goal setting section. Health and Physical Activity should be a lifelong commitment, not just for the span of class time. Teaching kids to set goals based on their health is an important skill for them to take with them into their adulthood. I found this tool on the PHE (Physical & Health Education) Canada website. HPE Canada is an organization created by health educators across Canada with the vision of having "all children and youth in Canada living healthy, physically active lives."
This is an easy to use tool but if you do all of the components of the test with a large group it could take a long time to complete. It can also require the active participation of the students and/or parents by asking the students to create their own accounts. These accounts can easily be linked to the teacher's accounts that they both have access to the student's results and progress. Furthermore, students can keep their account and continue to track their improvements outside of school.
